《涎腺良恶性肿瘤多模态超声声像图特征对比分析》为作者:高杏娜最新的研究成果,本论文的主要观点为目的 观察涎腺良恶性肿瘤多模态超声声像图特征,找出两者可能存在的鉴别点。方法 将121例患者的病例分为良性组和恶性组,分析比较良、恶性组多模态超声图像特征。 结果 常规超声各参数差异均无统计学意义(P<0。05),但将形态细分后,良性组以分叶或(椭)圆形居多(76。8%)、恶性组以不规则居多(66。7%),差异有统计学意义(P<0。05);良性组0-I级血流出现的机率高于恶性组,但两组血流均以Ⅱ~Ⅲ级居多,差异无统计学意义(P>0。05);超声造影增强后界线、增强环、增强后体积、灌注方向、峰值强度(PI)、上升支斜率及下降支斜率(绝对值)差异均具有统计学意义(P均<0。05),其中多形性腺瘤造影后灌注缺损区明显增加(24。0%vs72。0%),出现大片状无增强区的肿瘤多见于良性。结论 常规超声在鉴别肿瘤性质方面仅形态具有特征性,联合超声造影后鉴别点增加,在一定程度上有利于提高鉴别诊断涎腺肿瘤良恶性。肿瘤若出现形态不规则、高增强、增强环断续或未见、增强后体积增大及界线不明确应首先考虑恶性可能,但要注意与嗜酸性细胞腺瘤的鉴别。 Objective To observe the characteristics of multimodal ultrasound images of benign and malignant salivary gland tumors and find out the possible differences between them。 Methods 121 patients were divided into benign group and malignant group。 The characteristics of multimodal ultrasound images of benign and malignant group were analyzed and compared。 Results There was no significant difference in the parameters of conventional ultrasound (P > 0。05), but after the morphology was subdivided, the majority of the benign group was lobulated or elliptical (76。8%), and the majority of the malignant group was irregular (66。7%), the difference was statistically significant(P<0。05); the probability of 0-I blood flow in the benign group was higher than that in the malignant group, but the majority of blood flow in the two groups was II-III, the difference was not statistically significant(P>0。05); There were significant differences in contrast-enhanced boundary, ring, volume, perfusion defect area, perfusion direction, PI, ascending branch and descending branch slope (P < 0。05), among which the perfusion defect area of pleomorphic adenoma increased significantly (24。0% vs 72。0%), and most of the tumors with large areas without enhancement are benign。 Conclusion Conventional ultrasound is only characteristic in the differentiation of tumor properties, and the increase of differentiation points after combined with contrast-enhanced ultrasound is helpful to improve the differential diagnosis of benign and malignant salivary gland tumors to a certain extent。 If the tumor has irregular shape, high enhancement, intermittent or absent enhancement ring, increased volume and unclear boundary after enhancement, the possibility of malignancy should be considered first, but the differentiation with eosinophilic adenoma should be paid attention to。不知是否符合录用要求,望您批评与指正。
