摘要 目的探究腹腔镜结肠癌根治术治疗老年结肠癌患者的疗效,分析其对机体炎性因子、血清应激指标及免疫功能的影响。方法选取2016年9月-2018年6月保定市第二医院收治的60例结肠癌老年患者的临床资料,根据手术方式不同分为2组:对照组(n=30)、观察组(n=30),观察组患者予以腹腔镜结肠癌根治术治疗,对照组患者予以开腹根治术治疗,对比2组患者炎性因子、血清应激指标及免疫功能情况。结果术后观察组的CRP、IL-17、TNF-α、去甲肾上腺素、肾上腺素与皮质醇水平均明显低于对照组(P<0.05),而观察组的CD8+、CD4+及NK细胞水平均明显高于对照组(P<0.05)。结论腹腔镜结肠癌根治术治疗效果与开腹手术无明显差异,但炎症反应较低,机体应激反应较小,对免疫功能影响较小,微创及美容优势显著,具有一定的临床应用价值。 Objective To explore the curative effect of laparoscopic radical resection of colon cancer in the treatment of elderly patients with colon cancer,and to analyze its effect on the inflammatory factors,serum stress indexes and immune function.Methods The clinical data of 60 elderly patients with colon cancer who admitted to the Second Hospital of Baoding City fromSeptember 2016 to June 2018 were collected.The patients were divided into control group(n=30)and observation group(n=30)according to the different operation method.The observation group was treated with laparoscopic radical resection and the control group was treated with open radical resection.The inflammatory factors,serum stress indexes and immune function of the two groups were compared.Results The postoperative levels of CRP,IL-17,TNF-α,norepinephrine,epinephrine,and cortisol of the observation group were significantly lower than those of the control group(P<0.05).The postoperative levels of CD8+,CD4+and NK of the observation group were significantly higher than those of the control group(P<0.05).Conclusion The therapeutic effect of laparoscopic radical resection of colon cancer is not significantly different fromthat of open surgery,but laparoscopic radical resectionhas lower inflammatory response,less stress response,less effect on immune function,and has significant minimally invasive and cosmetic advantages,which has certain clinical value.
