血液透析护理人员血液暴露实时检测方法的建立与临床应用

摘要 目的建立一种血液透析护理人员在患者血路操作过程中血液暴露的实时检测方法,为提高护理人员手卫生和手套更换的依从性提供科学依据。方法改良粪便隐血检测方法的步骤,其检测血液的原理为匹拉米洞半定量检测法。采用匿名制随机抽样检测方法,使待检人员(刚完成患者血路操作未脱卸手套)五指并拢,在对应的检测框内(已滴加显色剂)用力按压数秒钟,并在规定时间内观察按压处的变色结果。同时观察碘伏、乙醇、免洗手消毒凝胶对该检测结果的干扰作用。结果 2019年9-11月间,对刚完成患者血路操作的护理人员进行186人次检测,109人次(含双、单手)手指处血液检测结果阳性,阳性率为58.60%(109/186)。109人次的阳性人员以单手的半定量结果分类显示,"1+"阳性率占19.35%,所占比例最大,"4+"阳性率占5.38%,比例最小;"2+"和"3+"阳性率分别为10.48%、10.22%。消毒剂对本检测方法无干扰作用。结论本研究提供血液透析护理人员在患者血路操作中,手部血液暴露的证据及血液暴露的概率,本研究证据支持血液透析护理人员在医疗活动操作中,应严格实施手卫生、及时更换手套这一基础性的感染控制措施。 OBJECTIVE To establish a real-time detection method for the blood exposure of hemodialysis nursing staffs during the operation of patients’ vascular access, so as to provide a scientific basis for improving the compliance of nurses with hand hygiene and gloves replacement. METHODS The procedures of the fecal occult blood detection was modified, and the principle of the blood detection method was the semi-quantitative pyramidon method. An anonymous random sampling test method was adopted to select person to be inspected,(the nursing staffs who has just finished the vascular access without removing the gloves) bring their fingers together, press hard in the corresponding detection box(the color reagent has been added) for several seconds, and to observe the discoloration result of the pressed place within the specified time. At the same time, the interference of iodine, ethanol, hand sanitizer gel on the results of fecal occult blood test were observed. RESULTS From September to November 2019, 186 nursing staff who had just completed the operation of hemodialysis patients’ vascular access were tested, and the blood test results of 109 people(including double or single hand) were positive, and the positive rate of 58.60%(109/186). According to semi-quantitative results of one-handed classification of 109 positive persons, the positive rate of "1+" accounted for 19.35%, with the largest proportion, and the positive rate of "4+" accounted for 5.38%, with the smallest proportion. The positive rates of "2+" and "3+" were 10.48% and 10.22%, respectively. The disinfectants had not interfere effect on this detection method. ConCLUSION This study provides for the first time the evidence of hand blood exposure and probability of blood exposure by hemodialysis nursing staff during the operation of patients’ vascular access.
