超声心动图诊断肺动脉型大动脉炎的价值

摘要 目的探讨经胸超声心动图诊断肺动脉型大动脉炎(TA)的价值。方法回顾性分析确诊为肺动脉型TA的患者58例,根据TA是否累及左右肺动脉,将患者分为TA肺动脉受累组38例和肺动脉未受累组20例。超声心动图主要观察指标包括肺动脉直径、肺动脉管壁厚度、肺动脉内是否存在血栓。结果肺动脉受累组在左右肺动脉直径、右肺动脉管壁厚度与肺动脉未受累组间的差异有统计学意义。超声心动图诊断TA肺动脉受累的灵敏度79.0%、特异度95.0%。超声心动图与金标准诊断的一致性检验:Kappa值0.719,P<0.001。结论超声心动图可以显示TA患者的肺动脉病变,以提示肺动脉型TA的诊断。 Objective To evaluate the value of transthoracic echocardiography in diagnosing pulmonary artery involvement in Takayasu′s arteritis(TA). Methods Fifty-eight patients diagnosed as pulmonary artery involvement in Takayasu′s arteritisretrospectively analyzed. According to whether TA involved the right and left pulmonary arteries, the patients were divided into the TA group with pulmonary artery involvement(38 cases) and the TA group without pulmonary artery involvement(20 cases).The main indexes of echocardiography included pulmonary artery diameter, thickness of pulmonary artery wall, and presence of thrombus in pulmonary artery. Results There were statistically significant differences in diameter of right pulmonary artery and wall thickness of right pulmonary artery between the pulmonary artery involved group and the pulmonary artery uninvolved group. The sensitivity and specificity of echocardiography in diagnosing TA pulmonary artery involvement were 79.0% and 95.0%, respectively.Consistency test of echocardiography and gold standard diagnosis: Kappa value 0.719, P<0.001. Conclusions Echocardiography can show pulmonary artery lesions in TA patients, to suggest the diagnosis of pulmonary artery involvement in TA.
